form for external bug reports

Formerly i used Google Forms for my bug tracking, later i decided that i want to run external bug reports via Tally.so, this form is currently very complex and i cant seem to make a neat GitHub issue template, thats why i will look back at Google Forms! The Google Form is back open at the same url:
