I think the private, Christian college I work for does have these questions on the application form.  They are used to flag 'at-risk' students, who then meet with the VP for Student Development before being accepted.  Our campus is drug-free & alcohol-free, so it's important to make sure that incoming students with a history are made aware of our policies and willing to abide by them.

1.       Have been convicted of a felony?

2.       Used illegal substances?
