Hello ACD! Happy New Year!!
Attached is the evaluation form for the OSH and other dormitory services.
Please read and comment on what should be changed or added to improve the survey.
You are given until Saturday, January 8, 2010 to voice out your concerns and opinions
para by Sunday I can include the changes suggested and upload it again here in our groups.
Tapos, you can print copies for the residents in your respective dormitories and distribute them para sagutan.
I'm not sure pa sa deadline ng filled-out forms. I will update you soon sa date na dapat matapos ang forms.

Ysa, may part na think dapat instead na thing. Tapos, 'yung NOT MET better yata na dissatisfied?
Dagdag rin ata ng ceiling lights other than table lamp.
Kulang rin ata ng question na about security or safety. Tapos measures ng dormitory about it. Pati yung sa first aid/medical supplies.
