Uploading Attachments to Contact/Client Forms using the Forms API

I'm trying to upload attachments to either the contact or the client using the Forms API and I can't get it to work. The rest of the data (like contact name, email, etc.) comes through just fine, but there's no attachment and I can't figure out what's going on. I'm assuming it's something with my format, but the documentation is pretty minimal on the attachments, so I haven't been able to figure it out.

So... for the company form...

I'm submitting contact_firstname, contact_surname, company_name and affiliation_email and those are all coming through properly.
For the attachment, I'm using "filepath_attachment_TEST" = "Interviews" and "attachment_TEST" = {Actual File}.
Interviews is the folder on the company record that I want to upload it to and TEST is the name I'm using for the file name.
