Dear Colleagues,

Following the finalization of contract process with health
management
organizations, two HMOs were selected namely Royal Exchange
Healthcare Ltd
and Zenith Medicare Ltd to handle the Health Insurance for
staff members.
Staff members are therefore expected to indicate their
interest in either
of these organizations.  Hence, please find attached
herewith the
following documents for your review.

1.    Details of Hospitals listed on each
HMO’s scheme
2.    Details of Health Plans/Benefits of the
two HMOs
3.    Staff Interest Form
4.    Enrollee Forms for the two HMOs

Kindly let us know through the attached Interest Form which
organization
you wish to be enlisted on and forward same through your
respective
Project Administrative Officers on or before Monday 8th
February, 2016.
