My name is Emily Tuttle and I am a second year graduate student in the
Master’s program in Speech-Language Pathology at Idaho State University-Meridian.

I am conducting a research study using an online survey. The purpose of
this study is to find out more about the factors that parents feel lead
to success with an AAC device or abandonment of the device by their
child. Gathering these important opinions will help educate teachers,
speech-language pathologists, AAC device manufacturers, and others of the
areas that need improvement.

CONSENT TO PARTICIPATE IN RESEARCH
Parents’ Opinions Concerning Success and Abandonment of AAC Devices

You are asked to participate in a research study conducted by Emily Tuttle,

1311 E. Central Dr., CSED, Meridian, ID 83642, 208-373-1728 from the
Communication Sciences and Disorders Department at Idaho State
University-Meridian. The primary researcher for this study is a student who
is using the results of the research for her thesis. You have been asked
to participate in this research because you have a child that uses an AAC
device. The number of participants in this research project will depend on
how many choose to take the Internet survey. Your participation in this
research project is voluntary. You should read the information below, and
ask questions about anything you do not understand, before deciding whether
or not to participate.

1. PURPOSE OF THE STUDY
The purpose of this study is to find out more about the factors that
parents feel
lead to success with an AAC device or abandonment of the device by a child.

2. PROCEDURES
If you volunteer to participate in this study, we would ask you to do the
following things: Use a computer to take the Internet survey using the
provided Surveymonkey.com link. Completion of this survey should take
roughly 10 minutes. Once the survey is completed, you are finished with
your participation in this study.

3. POTENTIAL RISKS AND DISCOMFORTS
The research procedures may involve risks that are currently unforeseeable.

4. ANTICIPATED BENEFITS TO SUBJECTS
The anticipated benefit for taking the survey is the opportunity to voice
your
valuable opinions. You have the right to
refuse participation in this research study.
5. ANTICIPATED BENEFITS TO SOCIETY
The benefit to society is that knowing parent opinions may help
speech-language pathologists provide services that are more sensitive to
family needs, thus strengthening AAC device implementation and success in
the future.


6. ALTERNATIVES TO PARTICIPATION
This research study aims to protect the identity of both participants and
non-participants.

7. PAYMENT FOR PARTICIPATION
You will not be paid or offered other benefits for participation in this
research
study, other than the knowledge that you are providing your opinions.

8. FINANCIAL OBLIGATIONS
There are no financial obligations for you to participate in this research
study.

9. PRIVACY AND CONFIDENTIALITY
Due to the recruitment methods for this survey, the members of the research
team
will not know who has chosen to take the Internet survey. By using
SurveyMonkey.com to host the survey, you will continue to remain an
anonymous participant in the research study. The anonymous individual data
will be stored
in a password-protected file on the personal computers of the members of
the
research team. Data will be kept for 7 years after an article is published
on this research.

11. WITHDRAWAL OF PARTICIPATION BY THE INVESTIGATOR
The investigator may withdraw you from participating in the research if
circumstances arise which warrant doing so. If you do not have a child that
uses an AAC device or if the child only uses sign language, you may have to
drop out of the research, even if you would like to continue. The decision
may
be made either to protect your health and welfare, or because it is part of
the
research plan that people who develop certain conditions may not continue
to
participate.

In the event of a research related injury or if you experience an adverse
reaction, please immediately contact one of the investigators listed below.

13. RIGHTS OF RESEARCH SUBJECTS
You may withdraw your consent at any time and discontinue participation
without penalty. You are not waiving any legal claims, rights or remedies
because of your participation in this research study. If you have any
questions regarding your rights as a research subject, you may contact the
