Blank php in design view

Hi,
Can anyone help.

I am using dreamweaver MX 2004 on the mac and trying to create an
emailable form. Completed all the steps through Web Form Factory no
problem. No error messages appear. I configure my 'configuration.php'
with my email address but when I upload 'configuration.php' and forms
folder containing the php files my form is blank. The html info is
there but it appears that once the php code has been put in front of
it the form visually cannot be seen on the page?? Any ideas.
