7TH ANNUAL WELLINGTON SHOOT-OUT JANUARY 11TH-12TH, 2014
SCHEDULE Be sure to check your schedule once again as we had some last-minute updates.
REGISTRATION / CHECK-IN Reminder that check-in will be Friday, January 10th at 5PM - 8PM.
* Deadline for online check-in is Friday at 4PM.
* There will be no Saturday check-ins.
Remember that the following documents are required for check-in.
1. Current Year, verified Player & Coach Passes with current photographs (Please have passes arranged in alphabetical order)2. FYSA Event Roster - 4 copies3. Medical Release Forms for each player arranged in alphabetical order (does not need to be notarized). If you already have completed medical release forms from your club, there is no need to use our form.5. Completed Guest Player Forms (as applicable). Guest player forms must be attached to the Roster and must be signed by the guest player's Club Registrar

Bert did we already signed for the medical release for this tournament, honestly I cant recall.
