recruitment and training procedure (questionnaire)

Friends, am preparing questionnaire on recruitment and training
process as the part of my research project. So, any one of you please
suggest some questions regarding to same? I have already prepared the
questionnaire but it was very basic. I want some more practical
questionnaire. the servay has to be done on top 300 companies.
Your suggestion will help me alot.
