Did you know that not taking medications correctly is the number one reason for being re-hospitalized? Gather your prescription medications & bring them with you to the brunch. Your parish nurse (me) and a nursing student (Alison) can review your medications and answer any questions you have about them. We can help you write questions or concerns about your medications to discuss with your doctor or pharmacist.
Throughout the county, the Parish Nursing Program assists with the "Vial of Life" program. This is a one-page health history that goes in a plastic medicine bottle in your refrigerator. A window-cling goes on your door, which tells emergency medical staff to get the vial and take it, along with you, to the emergency room. The health care team knows your important history quickly, without any delay while waiting for the computer. You'll receive the vial, help completing the paper, the window-cling, and a reminder card that I will send you in a year to remind you to update the information in the vial.
Have your blood pressure checked, and create a card to monitor any changes from month to month.
