Caring Connections and Advance Directives

 

Caring Connections, a program of the National Hospice and Palliative Care Foundation, provides family caregivers with many valuable free resources.  You can view, download and print advance directives specific to your family member’s state of residence.  (Advance directives are legal documents specifying a person’s wishes regarding future medical care and designating someone to make health care decisions for him should he become unable to do so.)

 

Caring Connections’ toll-free HelpLine (1-800-658-8898) offers assistance with understanding these advance directives. 

 

Caring Connections, supported by a grant from the Robert Wood Johnson Foundation, aims to engage people in making informed decisions about their end-of-life care.  Applying the acronym LIVE they encourage people to

 

Learn about options for end-of-life services and care

Implement plans to ensure wishes are honored

Voice decisions to family, friends and health care providers and

Engage in personal or community efforts to improve end-of-life care
