Wen,
The forms are necessary to check into Canada at the remote border crossing station. I would suggest that you put Quetico Provincial Park for location as that is the most accurate for your crews. The crew advisors should carry these with them for the border crossing and may be asked for them again to check back into the US. Canada would probably work, Ely, MN may be a problem as that is where they will cross the border from, not to.
I know one of your crews plans to stay in the US. That crew does not need this form.
I hope this helps, let me know if you have any more questions.

Thanks for your email. Our crew number is "E072813BCHI".
What is the parental release form for? It's not for the CANPASS RABC permit application, is it?
Do we have to mail the parental forms to you before our trip? Or do we give the forms to our adult leaders and have them bring with them?
What should we fill in for the "Location" field on the parental release form? Some parents filled "Ely, MN", some parents filled "Canada". Are these all ok?
Thanks a lot for your help!
I understand you had a question about a parental release form. Please let me know the question and either the crew number of troop and dates so I can help.
