Subject: New HealthBridge Discharge Summary  Plan of Care Section

Hello,


Please be advised that on July 1st, 2014 the newly developed "Plan of Care" will be available  for use in the current SUNY DMC electronic Discharge Summary. The newly added section will facilitate the physician HealthBridge "Plan of Care" charting while complying with the newly mandated Meaningful Use regulations which require all patients upon discharge, to have a Plan Of Care documented in their Discharge Summaries.  

Trainers will be circulating on the units to assist you if needed.  Please call ext. 4357 option 8 if you require assistance.
