Doubt regarding Exp Ver FormPart-3

Hi,

 

I am finding it difficult when filling Exp Verification Form-Part 3 for the PMP exam.

  >> Its said to summarise the project tasks that I led and directed.Candidates are required to summarise deliverables.Attachements ( ie scope of work documents ) will not be accepted.  >>

I am from the IT field.We have set of documents for Initiating phase , ProjectPlan for planning phase , SourceCode for Executing Phase and Tracking metrics and Testing results for Monitoring phase and RunBook and Sustain Documents for Closing Phase.These are the general deliverables for any of our projects.

Are these things supposed to be entered in this form.

Can anyone tell me an example so that I can get clarity on this.

Please correct me if I am wrong.
