BT call set up fee

Looking at page 3 of the document

it appears that the BT call set-up fee now varies, depending on the plan you are on (and perhaps also the type of call made). So we will need to modify our sites to say that the BT call set-up fee can now be up to 15p (for those on the new BT call plans introduced in April 2013).
