[MT-L] Modified MORSE Risk Scale

Hi everyone,

 

This sounds like a simple task but not so.  I am looking for a referenced copy of the MODIFIED MORSE Risk Scale.  We need to validate our scale and I cannot come up with a copy of it!!

 

If you have a copy of the scale with the reference I would greatly appreciate a copy or a link.
