[MT-L] Lab ORTHO VITROS 5600

Our Lab has just purchased a new ORTHO VITROS 5600 to replace 2 other
instruments. Has anyone had any experience connecting and interfacing this
instrument into Meditech Magic Lab? I am told this is a new interface for
Meditech and they have no information available. I am also told VIROS now
has the capability of doing a direct RJ45 Ethernet LAN connection vs the
Serial to RJ45 to the DIGI Term Server. Specifically what did you use to
make the physical interface connection (DIGI Term Server or Direct LAN
Conection) and how did you define this to Meditech?

We are on Meditech Magic 5.6 SR3 PP15.

We're Magic 5.63 and were told the same last year. We elected to use a serial to RJ-45/Digiserver solution. That will interface the instrument. If they also got the middleware system (and want to use it) for QC, etc., an HL7 ($20,000) interface will be required.
Information should be on the Meditech website (http://meditech.com/analyzers/instruments/device.asp?platform=MG&device=VITROS-51&server=DIGI
settings should be the same as the Vitros 5,1 (even though they are different interfaces) EXCEPT I believe the prefix codes for sample types are different - serum is now 5, plasma 6, etc.,.

With a DTS we received with our 5.63 update, on the instruments, the frame size is set to 240 (it was set by default by Ortho to 64000).
There was some additional tweaking for some of the QC (set up additional duplicate channels to recognize either serum or plasma).
Also be aware of an issue we discovered and reported to Ortho - if the instrument is set to transmit results upon completion, by test, it is possible for the repeat value to transmit, then be overwritten by a transmission of the original results (test being repeated completes before the original value is released). Disabling the feature of releasing results to the interface by test when completed defeats the improved turnaround time. Each Lab has to evaluate whether they want a diluted and repeated value overwritten by the original result. Not an analyst decision.
Also, watch for the < or > characters - you won't see them. The instrument sends a 4 or 5 , depending on the character and it's not handled by the current interface. We elected to set the analyzer type to flag the limits to prevent staff from accidently releasing an incorrect result - i.e. chloride sent from instrument (<50 on instrument) shows in the analyzer batch as 50, so we have our flag set at less than 50.1 to display <50 in Meditech.

Overall, wasn't a bad interface - two weeks from delivery to validation.

We are actually replacing an ORTHO VITROS ECiQ. Is this the same interface?
I could not find a standard serial to RJ45 connection that would work, so I
created my own home-made connection by trial and error until I found the
correct pin out combination that worked! I was hoping with the new 5600
instrument that a straight RJ45 connection would resolve both the interface
and e-connectivity issue with 1 single LAN connection.

We also we told the same and learned after nearly 1 full week of trying
to get it to work with the Ethernet connection that Meditech MAGIC is
not able to act as the Server for the Ethernet connection. We had to go
w/ the RJ45/Digi solution as well.
The point that Paul mentions below about the frame size to 240 is
correct.

For the topic of the Dilutions. We are working around this with a Lab
Calc to blank out the result if it equals our upper reporting limit.
This blanks out the result and waits for the diluted value to be sent
over from the analyzer. This works as long as the calcs are built for
each analyte to each specific high end reportable range (and has to be
exact for decimal places). Example:
IF{CA=">14.0" [f lres delete]...
The </> signs can be sent and is a setting on the analyzer...but watch
out if you do send them. Meditech recognizes the "<" symbol as a alpha
character which is always greater than the high end and if your
calculations do not account for it, a result of say <10.0 will report
out as >50.0 since "<" (an alpha character) is greater than the high end
reportable (in this case 50.0). Calcs can be adjusted to be looking for
it. If anyone has a great idea to correct this, please share back with
me.

Thanks for your responses, you have probably saved us weeks of work! One
question: where did you get the serial to RJ45 patch cable? A few years
back when I did this for the ECI, I was not able to find an "off the shelf
standard patch cable". After ordering, testing, and returning several, I
finally created my own with a Radio Shack kit one pin at a time!... Never
again!
