[MT-L] NPR Value/Keywords

Hey L-listers,

I created an NPR report but I am trying to standardize the select
fields. I what to choose about 25 discharge dispositions automatically
and behind the scenes so the user does not have too and can't make the
selection. I was using a LI but I can't figure out how to select more
then one item at a time. Any thoughts?

Hello,

Our document imaging application RASi collects patient photographs into
a
master patient index. These are accessible from date of collection
forward
to embed on patient forms and for patient identification processes.

The solution is also integrated with Meditech.

Hello,

This is a simple process for customers of RAS (Report Automation
System).

Our RAS engine will parse each page (or subsection) of a document and
attribute (tag) the physician related to the patient account to the RAS
database. Each page/subsection may then be automated for delivery, i.e.
Fax, email (either email the entire document or merely a link to the
entire
document), printing, text message etc.

Hi,

We are c/s 5.64 site using Forward Advantage to distribute our DI
reports to providers. We have a mix of methods of distribution. Some
providers want to be faxed, some want to view electronically, and others
want printed copies. The printed reports are being printed to the DI
department but we are having an issue sorting out whose copy of the
report this belongs to. Short of having a list of those providers who
don't have fax, I don't know how else to simplify the sorting, or tag
these reports. Does anyone have a solution?

Our policy reads once a day, but nurses are encouraged to do it every
shift (generally the 8-12hr shifts) so that it gets done once a day.

Hi all! We are currently redesigning our nursing documentation. We
have designed a new Care Plan Intervention that allows the RN to
document the status of the problem (improved, declining, etc) and also a
free text box to write what they are doing with the patient in regards
to that problem. Our Clin Specs have written into a new policy that RNs
must address/document against every problem in the Care Plan every
shift, even if they just come in for a short shift (like 4 hours). The
nurse superusers on my project team feel this is way too much
documentation. I am looking for what other facilities require in terms
of Care Plan documentation. We will be having a meeting with management
soon, and I'd like to be able to present what other facilities require.
We are Magic 5.63, but would be interested in all replies!

Just curious, if Medicare audits an account and requests documentation,
what is your process when the account number billed does not equal the
one with the documentation? How do those pulling documentation know
which other account to look for the documentation on?

We leave all documentation, reports, orders, etc... On the appropriate
account. Medical Records runs a 72hr report and forwards accounts that
should be "linked" to our BAR Charge person. He then reverses charges
off one account and rebatches to the appropriate account. This way all
charges are on one claim that gets submitted to Medicare and yet
everyone else can find what they need in the EMR.

I would greatly appreciate learning how other sites deal with the issue
of having two accounts opened for one Medicare patient with the same
date of service. We are dealing with signed reports and results posted
through interface having to be moved to one account. What a mess and a
manual data entry nightmare. I can't imagine what will happen when we
go up on nursing documentation and this occurs. If you have a process
in place to successfully handle this I'd appreciate hearing from you.

Below is an excerpt from our policy. Of note is the sentence "via the
process intervention screen". Because Nursing is reviewing the PI screen

every shift, adding, completing changing interventions, she is updating
the care plan. We have used this method for 10 years and have passed
several CMS inspections with no issues.

The registered nurse may choose an appropriate plan of care from the
established NUR Plan of Care dictionary and must review and
individualize it to meet the patient?s care needs via the
process intervention screen. Upon completing the initial plan of care,
the system will record the name and title of the staff person completing

the process.

Editing the Plan of Care
The plan of care is reviewed at least every shift for appropriateness to

patient?s care and condition via the process intervention screen.
Review of the plan of care includes continued individualization
regarding existing interventions,additional interventions, text and
frequencies by the nurse caring for the patient.

Hi Bob,
Can you put a question on the Meditech L-list to find out how other
sites are handling the billing of single dose vials for outpatients?

This won't be easy to set up. Every single dose vial that we use for
outpatients will probably need a separate drug dictionary entry that
charges for the exact dose, plus another drug dictionary entry that will
charge for the exact waste with the JW modifier. For inpatients, (and
non-Medicare outpatients?) we'll want to continue the current practice
of charging for the whole vial. That could mean 3 DD entries for every
drug that this applies too. I'm wondering if other sites enter 2 orders
for each patient, one for the dose given and one for the amount wasted.

Good Morning Everyone,

I have a question from our Scheduling area. Has anyone out there come
up with a good way to track all the appointments associated with an
authorization so that additional appointments not authorized are not
booked? For example: A patient is authorized for 5 visits to PT for a
particular modality. How do you make the sure the patient doesn't get
booked for a 6th without obtaining more authorization? How would someone
in your facility know if this is visit 2 or visit 5 of the authorized
visits?

We are using RCR accounts for therapy, but we discharge all accounts at
the end of the month and re-register those with outstanding appointments

Any input appreciated. We are just trying to improve our processes and
one of you always seems to have a creative idea.

Same process, we use the BAR comment field to note which account charges
were moved to. This appears on the main Process Account routine, so is
easy to locate.

We leave all documentation, reports, orders, etc... On the appropriate
account. Medical Records runs a 72hr report and forwards accounts that
should be "linked" to our BAR Charge person. He then reverses charges
off one account and rebatches to the appropriate account. This way all
charges are on one claim that gets submitted to Medicare and yet
everyone else can find what they need in the EMR.

through interface having to be moved to one account. What a mess and a
manual data entry nightmare. I can't imagine what will happen when we
go up on nursing documentation and this occurs. If you have a process
in place to successfully handle this I'd appreciate hearing from you.

We opened a task with MEDITECH regarding the new JW codes from CMS. They
are aware of the issue. For now, it appears no coding has been done as
of yet, but this new system goes live on July 31st of his year, so they
better get to it.

Several years ago we changed from headers on Departmental Reports to NPR

fragments with bar codes. We have had continuing problems ever since
(task open since 12/08).

First we encountered the error message on the top of page 2 on some
reports (Printing Error - Contact Data Processing). The fragment
printed, followed by the error message and that was it for the report.
We have had problems with printing both to LOCAL and Meditech printers.

This clear up with the 5.63 upgrade.

Now we are encountering pages that print one line on Discharge
Instructions and we end up with 9 pages on a 6 page report. I thought
that this was due to multiple sections listed within the report so I
combined all sections. This seemed to work for a brief time until we
upgraded to 5.63. Now we are back to the extra one line pages.

Meditech states that it's the fragment but has no clue how to fix.
Medical Records wants the bar codes on the reports for scanning (patient

account and Form IDs). This issue is back to Meditech Development which

we all know is a endless, black rabbit hole.

Has anyone experienced/solved this issue?

I started this discussion a couple of weeks ago, so you might want
to check the archives for responses. The bottom line is that Meditech
does not have this functionality nor will they have it by 7/31. After a
lot of discuession here and on the ASHP informactics lits serve, it
appears that CMS is leaving it up to the fiscal intermediariers how to
handle this requirement. I called our billing department last week and
asked
that they set up a conference call with Highmark, our FI, so hopefully
we can find out some answers as to how to meet this new requirement.
Also, I suggested that ASHP start advocating for a delay since from
what I gathered off of the ASHP informatics listserve, there is not a
single vendor out there with the functionality needed to address this
issue (Epic seems to have a partial solution, but from what I see, it
isn't a seemless solution, which is what we need).
I would suggest that everyone keep on Meditech for a solution (and give
them you ideas as to how this would be accomplished), plus work with
your professional organizations to get this delayed (as we did with the
NDC requirement).

We are Magic 5.63 and I was working on the Meditech KB article 20522
incorporating ADR's to our PHA.PAT view. I got it working, however it
doesn't pull in the ADR comments. Has anyone got this to work? I found
some older documentation but it the syntax won't translate to this
version.
Any thoughts?

Julia, I would guess most of us have just been sitting here with the
deer-in-the-headlights-stare, just trying to digest what Charlie is
trying to deal with.
I am guilty sometimes of communicating off-list with some members, but
try to share solutions. Even those of us more experienced (or is it just
older, after working with Meditech many years?) hesitate to toss out
suggestions or comments, for fear of chastisement (or we use honest
language not appropriate for the tender ears of some list-members).
As you indicated, we need to remember not only to respond to the list,
but listmembers probably need to be reminded to search the archives, and
other resources, for potential solutions before posting their questions,
unless a new regulatory issue, like Charlie had asked about.

I would also like to share another tool some of us forget to use when
we're really busy fighting numerous issues - Meditech's Product
Development Resource Center Documentation Search, where you can search
the DTS'es available between your current Meditech version and the
latest available. I perform the search on "any" (like the 653 currently
developed for LAB since our 5.63 update), then use a search through the
browser to find potential matches. I have discovered potential solutions
to at least 3 of our Meditech and LSS issues within the last couple of
weeks. It's a lot faster than waiting for support to do it (even though
that's part of what we pay them for).

that they set up a conference call with Highmark, our FI, so hopefully
we can find out some answers as to how to meet this new requirement.
Also, I suggested that ASHP start advocating for a delay since from
what I gathered off of the ASHP informatics listserve, there is not a
single vendor out there with the functionality needed to address this
issue (Epic seems to have a partial solution, but from what I see, it
isn't a seemless solution, which is what we need).
I would suggest that everyone keep on Meditech for a solution (and give
them you ideas as to how this would be accomplished), plus work with
your professional organizations to get this delayed (as we did with the
NDC requirement).

charges for the exact dose, plus another drug dictionary entry that will
charge for the exact waste with the JW modifier. For inpatients, (and
non-Medicare outpatients?) we'll want to continue the current practice
of charging for the whole vial. That could mean 3 DD entries for every
drug that this applies too. I'm wondering if other sites enter 2 orders
for each patient, one for the dose given and one for the amount wasted.

Set up a "start" macro like this:
1^/DISP["FIELD1"],
1^/DISP["FIELD2"]

until you have them all entered. Make sure the last one does not have a
comma after it. All the other should as above.

On page 2 selects add the following:
dis.disp (substitute the correct name) LI /DISP

On page 3 add a footnote as follows:
AL START start (you can substitute whatever name you called your start
macro)

That should do it.

If you are not comfortable with macro's I edit the screen and add them
in there, then teach the user to copy/paste.

several ways. I have the report working, but for fun I am going to go
play with a macro.
