My implementation team would be interested in knowing when your facility
requires drugs to be co-signed on your EMAR. Below is the list from our
policy of what we consider High Risk categories/Meds. Please indicate
with a Yes/No if you require co-signature on EMARs.
Send the responses directly to me and I will post the total results Monday.

Class/Categories of Medications
• Adrenergic agonists
• Adrenergic antagonists
• Anesthetic agents
• Chemotherapeutic agents
• Epidural and Intrathecal medications
• Glycoprotein IIB/IIIA inhibitors (Aggrastat, Integrilin)
• Inotropic agents
• Liposomal forms of drugs
• Moderate sedation agents (IV midazolam, PO chloral hydrate
• Narcotics/Opiates
• Neuromuscular blocking agents
• Thrombolytics/Fibrinolytics
• Total parenteral nutrition solutions
• All pediatric medications

Specific Medications
• Amiodarone (intravenous)
• Colchicine injection
• Dextrose (hypertonic - 20% or greater)
• Heparin unfractionated, LMW heparin injection/cocktail
• Insulin (subcutaneous and intravenous)
• Lidocaine (intravenous)
• Magnesium sulfate injection
• Methotrexate (oral: non-oncologic use)
• Potassium chloride (concentrate for injection)
• Sodium chloride (hypertonic - greater than 0.9%)
• Warfarin

Do you require Nursing Students to co-sign any or all drugs?
