[MEDITECH-L] Fall Risk assessments

We are looking to see what other facilities are using for gathering fall
risk information on your patients. Are you just using one intervention
and documenting on that, what type of information are you asking, etc.?

Hi Marie,

We do a fall risk screening upon admission and every shift, and if
appropriate document within the intervention any precautions that were
implemented.

I have attached screen prints of the screening and precautions.

Here's what our nurses gather on the initial assessment.  It's set up to calculate a value for each answer, default the total for the assessment score, and then default the risk factor in relation to that score:

Since the topic of Fall Risk has come up, I have another somewhat
related question.

Does anyone have either a screen or canned text for documenting a fall
when it happens? I am suppose to mock up a screen and/or canned text for
consideration of our documentation committee and I have to admit, I am
having a hard time deciding how to proceed. There are so many variables
for a fall. Almost think it should be a screen as it might be easier
than trying to compose a canned text that would work for assisted falls
as well as unwitnessed.
