In OM (OE), we currently have only have STAT, Routine, Timed and Daily as options for order priorities for Lab tests. We are going live with CPOE later this year. The providers are concerned that there is no “Now” priority available to them. In our Lab, Routine means that the patient will be drawn within an hour (“Now”) and STATs are drawn, well, STAT. We do have phlebotomy handhelds so orders are received by the phlebotomists real-time. I am very apprehensive about adding “Urgent” to the order options as several sites I have spoken with state that Urgents are just lumped in with STATs and really just muddy the waters. This issue is going to be house-wide as we would want the priority options to be the same for all departments (Radiology, Cardiovascular, RT, etc).
Any advice for us, whether it be pro or con?

I’ve worked with Meditech in 2 different hospitals and we use the Urgent priority in both. It’s as close to Now as you’re likely to get. We had no issues with it.

We encountered the same thing at our facility. The providers wanted different frequencies than the standard system offerings from Meditech. Also, we are using ProVation for evidenced based order sets and they offer a whole lot of other frequencies that don’t really relate to Meditech (ie. In AM, In PM, etc.). While you can create different frequencies in the MIS directions dictionary, any custom frequencies/directions are treated as ROUTINE frequencies which for lab means they fall into the next routine collection batch. Another side effect of custom directions is they are treated as series orders which in many cases could be a series order of one…..
We changed our wording on paper order sets in preparation for CPOE to Urgent @ _____(date/time) to address the NOW type orders. We also added the option of TODAY and will be removing the Routine option in the near future. It was the best we could come up with to make the ordering frequencies more intuitive for the providers.
