There seems to be some support for using S/Key with ssh built into
avpn, but unfortunately it is not working on my system. This is also
unfortunately not documented (not a strong point on this product :) ).
Can someone point me to some documentation or troubleshooting help?

Thanks,

You can specify which identity file should be used when connecting to
particular ssh server. To do it, select ssh server, then go to
"Options" tab. You should see all your identity files listed in
Identity pop up menu (***)

*** AVPN will automatically discover all identity files in ~/.ssh
(You should be able to see them under "localhost" in resource tree.
In case you are storing your identity files in different location you
will need to add these files manually.
