The linked VAA report discusses how to capture and preserve all this provenance and structure in TIU notes (which is over 85% of the content of TIU notes!) that is otherwise lost - all with no modification to VistA or CPRS.

This is outstanding. I’m working with a VA group developing an AI summarization pipeline for VistA data using vectordb off the embeddings of notes. As you note, variably a large amount of data in notes is structured data better gathered elsewhere in VistA for RAG. We developed some NLP to attempt to automatically dedupe and scrape boilerplate info in notes but it’s clunky. I’d be interested to hear more about this in 5.4.1:

“Use domain knowledge of VistA’s note types, templates and OBJECTS to annotate each note after the fact.”

I think page 18 of the documentation shows how important reminders are for care at the VA, and to my knowledge, there isn't anything comparable in Cerner where reminders are added frequently at no additional cost and users can write reminders and reminder dialogs as well as the reminders team.

Sure, but the missing feature of existing VistA Clinical Reminders is that it doesn’t fleece the taxpayer for billions. Obviously, a fatal design flaw that only the most absurdly expensive solution can fix.
