Take the HOPE Collaborative Member Survey!

Hi everyone,

 

As some of you may know, the evaluation committee has been working to develop this year's Collaborative Member Survey.  The role of the evaluation committee in the planning phase has been to assess the internal functioning of the collaborative in order to help us strengthen our ability to do work together. 

 

The survey is one of a number of ways that the evaluation committee is collecting information about what is working well in the collaborative and in what areas we need improvement.  The survey is available in paper form as well as electronically.  If you haven't filled out a paper version of the survey, please take 15 minutes to take the online survey here:

We hope that ALL HOPE members will fill out the survey, either on paper or through the linke above.  The information collected in the survey is confidential.  The evaluation team at ACPHD will summarize the information we collect and present the results to the Steering Committee later this summer.  We hope this information will help the collaborative to strengthen it's internal structure to allow us to be the best Collaborative we can be.
