Dear Stakeholders,

The Franklin County Health Department is beginning the process of selection a Priority Health Issue for our 2027-2031 Maternal & Child Health (MCH) contract, and we are seeking your input. 

We invite you to complete a brief MCH Priority Selection Stakeholder Survey. Your responses will guide us in identifying the maternal and child health issues that are most pressing, achievable, and aligned with the needs of families in our community. Feedback from community partners and stakeholders like you is critical to ensuring our priorities are data-informed and community-driven.

Survey Title: Maternal & Child Health (MCH) Priority Selection Stakeholder Survey

Purpose: To support the selection of a priority MCH health issue that will guide planning and programming for the FFY 2027-2031 contract period.

This survey should take approximately 5 - 7 minutes to complete.
