Jamati members can now apply for Long-Term Care at Generations Toronto through Ontario Health atHome.

When speaking with Ontario Health atHome, Jamati members may indicate Generations Toronto as their preferred Long-Term Care choice and state that they are *Ismaili Muslim*. Members who are already working with an Ontario Health atHome Care Coordinator may also wish to review or update their preferred choices. 

Eligibility and placement are determined by Ontario Health at Home as part of their assessment process.
