How about a long quest. I used one where a newer God know as the
shadow night His theme being protector in the dark. Think Paladin/
Thief or Cleric thief as priest. Have a high priest contact them and
let them know something from legend called the vault of the dead has
been opened and the ponly wa to close it is to retrieve the Totennicht
Bladeand deliver it to the Paladin it has been alled for in your
world. Make the sword as far as he map of your world will let you
take them and have them lead around by the old gate system the shadow
Knights have found. This system of course lead from one problem to
another until they get to the sword at last and one of them is
suddenly tapped to be the weapons avenger. It changes into the
characters favored weapon in shape
and off they go to close the vault. In my world the only way to close
it was to kill the one sho opened itand he was of course a powerful
necromancer. He has been throwing obsticles in front of
them the whole time and will make sue he and his minin choose the time
and the place of the final battle.
Hope this outline helps I left it to be fleshed out using your world.
If you need a world you can use the flat earth idea I am currently
using. The flat earth is the prototype prime material plane made
simple and connect to the elemental planes directly. The world has an
edge and you can sail of fall off. It also makes a great scene for the
final battle. The old man rises from the edge of forever flanked by
his undead air elementals and the ground before you erupts as his
undead earth elemental surface and prepare to attack. The ground
shudders even more as dead hands come thrustinging out to grab at you
and the old man laughs. This will get what is known as there
undevided attention. Adjust the undead to the power level of the party
and have fun.

Thank you pgr, for at least typing in, besides Me and Abrigon. This
idea has great potential, and I like a variant of this theme,
Combine the three-skull skeletal image with (In D&D Terms, Lich (as in
coming back after destruction, disintegration, etc.,) & Death Knight,
adding the True Necromancer (Cleric3/Necro.3/True Necromancer14,
+Clr4/+Necro4 and 280HD of minion-cohort & lesser lackeys, and adding
Thief21 & Monk21 (extra two skulls, not subject to crushing at 2/3 &
1/3 of hit points as par Skull Lords)
Yeah, the master villain that comes back time & again after defeats,
and a phylantry that must be found to end Mr. Boney permanently. Lots
of mayhem in that. However, after nine times crushing that guy gets
too frustrating for Players (& PCs) and off the edge (and away from
the gametable) means that the third time it ends permanently for Mr.
Boney. Basically having a few escapes, but three times crushing him.
Perhaps the artifact is in a dagger form instead of a sword?
