We are excited to share that 706 applications have already been submitted successfully. | | We are aware that some users are experiencing errors when creating or submitting IFSP applications. The cause has been identified and a fix is actively being applied. | | In the meantime, if you encounter an error, please try again later. We will notify you as soon as the update is fully applied and submissions are working as expected. We apologize for the inconvenience and appreciate your patience as we work to resolve this quickly. | | The Funding Portal will be open for 30 days, so you will have plenty of time to complete your application once the issue is resolved. | | IFSP-Funding Application Schedule (Fall 2025) | | First day to apply: October 1st at 9 a.m. | | Last day to apply: October 30th at 11:59 p.m. | | Before you begin, here are a few reminders: | | - To be eligible to apply, all applicants must be on the Developmental Disability Waivers Waiting List and be living in their own home, or in their family's home.
| | - Applicants with a priority 1 Waitlist status may apply for up to $1,000.
| | - Applicants with a priority 2 or priority 3 Waitlist status may apply for up to $500.
| | - You will NOT need to know your Waitlist priority number in order to apply. When you log in, the application will show you your priority number and how much you can apply for.
| | To access the application, please visit this link: | | IMPORTANT: We are approaching the card expiration date for funds that were issued in January 2025! IFSP-Funding recipients must activate, utilize, and/or withdraw the funds from their Way2Go debit cards. Failure to activate these cards may impact an applicant's eligibility for future funds. For assistance activating a card or obtaining a replacement card, please call Conduent's automated customer service toll-free number at 800-961-8423, or call the My Life, My Community toll-free helpline at 844-603-9248. | | For general assistance with navigating the IFSP-Funding Portal and application, please contact our toll-free My Life, My Community helpline (operated by SeniorNavigator) by phone at 844-603-9248. | | The helpline is there to support you!
