Updated Objective Written Documentation (OWD) Form, February 2024
An updated version of the Objective Written Documentation (OWD) Form (linked below) is available to providers of services under any of the DD Waivers (other
than Sponsored Residential who use the Family Member as Sponsor Provider Supporting Documentation Form) seeking to
hire family members who are living under the same roof as the individual receiving services. This form provides guidance to providers and family on meeting the required criteria when live-in staffing selections are requested. The OWD form is an optional form
and is not required by service authorization.
The definition of OWD per DD Waivers regulation 12VAC30-122-120: “… Payment shall not be made for services furnished by other family members who are living under
the same roof as the individual receiving services unless there is objective, written documentation, as defined in this subsection, as to why there are no other providers available to provide the care. Such other family members, if approved to provide services
for the purpose of receiving Medicaid reimbursement, shall meet the same provider requirements as all other licensed providers. "Objective, written documentation" means documentation that demonstrates there are no persons available to provide supports to the
individual other than the unpaid family/caregiver who lives in the home with the individual… “
The use of this form will not guarantee automatic approval of these types of requests, but will aid providers in capturing what is required
for OWD.
