Virginia Informed Choice Form (VIC) and
Service Selection Guide
We are excited to share the updated Virginia Informed Choice (VIC) Form and accompanying Service Selection Guide for the DD Waivers. These tools are designed to document an individual’s informed choice of services and providers. They were developed collaboratively with individuals with developmental disabilities (DD), families, community services boards, and providers.
What’s new?
The updates focus on simplifying the form and ensuring individuals and families have the information they need before making decisions. Links to both documents are provided below:
