Hi, guys.  Since I'm not on gmail, I can't access the distribution list for this committee.  I'm attaching a word document with a draft questionnaire.  It's modeled on those I used to use for  consumer research with the food industry.  Can you forward it to the group and ask  for comments, suggestions, corrections, etc??  If there's some other way I should do this, just let me know.
