Margin note in Rosais, Sao Jorge Baptism

Bottom right hand baptism for Manuel.  Can anyone please tell me what it says in the margin?  The Priest recorded the baptism with pai incognito but then changed it. Does the margin note explain this?  Thank you.

I think the note says "modou no crisma  [??] de [Simão Manuel] (I'm not sure about the bracketed words).

If so, then he changed his name to Manuel during the confirmation sacrament.  It does appear the "M^el" may be overwriting something else [Simão?].  That may be when his father's name was added to the record.  His parents didn't marry until 9 years later after the baptism in 1799.
