Faxbox Sending SMTP to Fax

I'm having difficulty setting up smtp to fax. I have successfully setup a Kazoo system with DIDs and can receive Fax to email. I have setup all the necessary settings with postfix to receive emails with TIFF/PDF attachments. I've setup offnet carrier and can send and receive voice calls with a hand set. 

However I can not seem to send a fax from kazoo using smtp -> fax. I have added the "fax" flag to the offnet documents. I get the following error in an email response...

Error : no available resources

Caller ID	XXX5551234   (XXXXXXXXXX)
Callee ID	unknown (originally dialed number)
Call received

Any suggestions would be helpful. I can successfully make voice calls and receive fax to emails.

you should check fax app logging for that request

How do I view the logging for the fax app? I can see logs for fax_util in the /var/log/kazoo/console.log.

Ok, I was able to turn on debug logging and found that the fax_worker is crashing....
