from October 1, 2023 to the dead line, December 31, 2023. Do NOT send them via the Post Office nor handle them to me.
1-This 4-page registration form: must be TYPED and not handwritten.
2-A scanned or picture of your US Passport still valid until Feb 2, 2025.
3-A most up to date picture of yourself in passport style (selfie is ok).
4-A non-refundable deposit check of $100, made payable to: De La Salle Institute and mailed to: Frère Phong, 1103 Maxey Court, San Jose, CA 95132. This deposit is for the Bonus Fund and is not counted toward the full payment.
All these four items are required to make your registration complete.
Tour Vietnam can not accept more than 45 people. First come, first served!
Service With the Poor in Southern Vietnam and/or Cambodia has no limit on the number of people.
