Dear campers and chaperons:

Please use the form attached to this email.  Please:

1. Copy the row that has your ID No. and name
2. Fill your passport and travel information
(a)   Check your name shown on your application form,
(b)   Fill in your name shown on the passport,
(c)   Fill in passport number,
(d)   Fill in arrival information,
(e)   Fill in the person who will pick up the camp in
another city or place after summer camp.
3. Send it back to me.

Please note:

A.      If you do NOT need pickup and drop-off services,
you do NOT need to fill out the arrival and departure
information.  Please fill in NO in the  pickup and
dropoff columns.

B.      If you DO need this service, you MUST provide the
information so that the organizer can arrange the
service.  I understand that many of you do not have
the information.  Please let me know you NEED the
service and provide detailed information as soon as
possible, but no later than July 4th.

C.      GO with CSAUS Group in Shanghai

If you leave and/or come back to Shanghai with the
group, we take airplane only.  We will fill out the
trip information for you.  Please fill the last 2
columns.
