Information for your trip

Hi All:

 

In order to arrange the trip for all Campers), you are requested to fill out the information listed in the attached file. We will use the information for the following purposes.

 

1.       Pick up Campers at airport or train station,

2.       Need correct information for buying flight tickets for Campers,

3.       Contact the person who will pick up a Camper in the destination place,

4.       Save our time for collect information.

 

Instruction for filling out the form:

 

You only need to fill out your information on the line with your name. Leave other lines blank. In other words, you also can keep the title line and your line, and delete all the other line that are for other campers.

 

(a)    Check your name shown on your application form,

(b)    Fill in your name shown on the passport,

(c)    Fill in passport number,

(d)    Fill in arrival information,

(e)    Fill in the person who will pick up the camp in another city or place after summer camp.

 

Please send your information to Xiaoyin Sun  (xiaoy...@yahoo.com ) and Nianfa Tang (nianf...@gmail.com) by e-mail before 8:00 am., July 4, 2007. Qiao Ban and Hunan Organizer need the information to arrange their schedule and work.
