exploiting error correction, scan not possible

Hello,
I'm currently working on an app, which exploits the error correction. I already read the qr specification and on page 45 I found the formula:
"e + 2t ≤ d - p
where:
e = number of erasures
t = number of errors
d = number of error correction codewords
p = number of misdecode protection codewords"
where p=0 since i'm working with 3-H codes. After creating 11 errors in the data codewords, I cannot scan, which should be possible according to the formula. Anyone know why?
