Over the handlebars due to braking hard, time trial bike which gives
reduced visibility (time such machines were banned from road use.)

He failed to notice the car in front slow down till it was too late,
swerved round it, braked hard, over the handlebars and landed head first
on the almost stationary car coming the other way.

I hope he was well insured.

How is it possible his helmet failed to save his life?

Indeed.
Had he survived we would have been told it was only because he wore a helmet.

No we should NOT ban them. It's nothing to do with you me or anyone else if somebody chooses to endanger THEMSELVES.

I was telling a girl in the pub about my ability to guess what day a woman was born just by feeling her boobs.
"Really" she said, "Go on then.... try."
After about 30 seconds of fondling she began to lose patience and said, "Come on, what day was I born?"
I said, "Yesterday."

I remember going over the handlebars a couple of times.
I am sure my helmet would have saved my life if I had been wearing one.
