Also, on a separate topic, have indexes been removed from ToyStore? Is there a recommended approach to doing something similar?

Indexes have been removed as in order to do them right (no race conditions, etc.) they really need to be a per store type thing. I didn't want to think about that yet, so I axed them.

Depending on the store you are using, you can just create them on your own and roll with it.
