swa default app issue on instances revisited

In 5.4.2 the SWA default app issue on showing instances is still there with console error:

I believe the layout message is actually just a consequence of opening(or resizing) the dev console - it shouldn't be causing any problems with functionality of the page.

The 'teamwork is not defined' message on swa default app is being looked at for 5.5.  The workaround Holger described previously should address it until then, if needed.
