David--There were three synagogues that operated on McCulloh Street:

1501-1505 McCulloh Street Chizuk Amuno Synagogue
2022 McCulloh Street Mishkan Israel Synagogue
2105-07 McCulloh Street Shearith Israel Synagogue

It appears that you are referring to Mishkan Israel Synagogue.

If you scroll down, you can see if this is indeed the one you are searching for.
More information can be obtained from the Jewish Museum of Maryland (see their
website). They published a book "Synagogues, Temples and Congregations of Maryland
1830-1990" written by Earl Pruce.

<<I came upon a Masonic Temple in the 2000 block of McCulloh Street in Baltimore
that appears to have been a synagogue. The central window has been partially
covered over, but the uncovered part clearly shows a Jewish star. And there is a
carving of the 10 commandments at the top. I'd love to find out about the history
of this synagogue...
