ViewMate translation request - Polish and Yiddish

I've posted two texts on ViewMate for which I would appreciate a
translation.

The first is an excerpt of just a few words and one name that I cannot
make out from a vital record (birth registration) found on JRI Poland,
written in Polish. It is posted on ViewMate at the following address:

The second request is a sample from a group of letters written in
Yiddish in 1924 from Galicia to relatives in Canada. The Yiddish has a
heavy Germanic overlay and a fair amount of religious phrases in Hebrew.
The handwriting is difficult, and I would appreciate it if even a part
of the letter could be translated.
