Select2 data from database

Im trying and trying but can´t get a working dropdown.

 

Can someone point me to an example of files (html, js & php) or code where

you load data from database with ajax,json,php and populate the select2 dropdown menu.

 

The examples with loading remote data is to thin.. i dont understand how to build the .php

You may have already reviewed this tutorial, but I was in the same boat about two weeks ago and was able to get a php/ajax/json demo working end to end with mysql.
