Since the release of RubyGems 2.4.0 and 2.4.1, there has been an increase of reports of gem installation issues [1] [2], all related to a platform-specific bug in RubyGems [3]
Until this bug is resolved, if you're a user of Ruby 1.9.3 (any patchlevel), do not upgrade to latest version, but instead limit it to 1.8.29.
At any point you can downgrade with the following command:
C:\>gem update --system 1.8.29
RubyInstaller 1.9.3 ships with RubyGems 1.8.28, so it is safe to upgrade to latest 1.8.x version (which is 1.8.29)
