Unless I am missing something obvious it seems like it is not possible

to reply to a (code review) comment which is not the last in a thread.

The "reply", "quote", "ack", "done" buttons are only available on the last comment.

If I want to reply to a previous comment, and quote some text from it, how do I do it?

Currently, I keep switching back to GWT UI for that.
