Did you updated that template? qrexec permission denied error suggests
R2 qrexec running on R3 dom0. Regardless of that, it shouldn't end up
with OOPS... Anyway if you could get some more details when this happens
(VM startup? some app startup? new window? close of some window? was
that netvm?), that would be really helpful.
