(Play/Java/HTML Question) I am looking for a form such as "DataEntry" or "CrossTab" or "Grid" in HTML/JAVA

hello everybody,

Can someone tell me a kind of **crosstab** or **dataentry** or **grid in HTML** having like this form and allowing to enter data in the middle (blank cases) ..

I will need to put the data entered into a database afterwards:

Example imagine we have this :

Sample 1
Analysis 1:
Parameter 1: (data to enter)
Parameter 2: (data to enter)
Parameter 3: (data to enter)
