Make developing extensions easier

I've created https://github.com/decibel/pgxntool with the goal of making
extension creation easier.

Currently it's focused mostly on providing a good base makefile that you
can include, instead of trying to craft your own. I've only tested it
with my test_factory extension so far, but since that actually builds
two separate Postgres extensions I know that part at least works.

Feedback and ideas on this are most welcome. I think the intimidation
factor of dealing with PGXS really turns off a lot of potential
contributors, and I'm hoping this will greatly improve that.
