[perl #33094] [PATCH] Clean Up POD Errors and Warnings

Here's a patch to clean up all of the POD errors I found and most of the
warnings, too. If no one objects, I'll commit it myself in a few days.

Some of the remaining warnings are legitimate differences of opinion
between podchecker and the sane world. Still, cleaner is better.
