RF RDSR from Toshiba (Canon) Ultimax

Hi,

 

From a Toshiba (nowadays Canon) Ultimax I get confusing high numbers on the total dose (10.000.000x more), also the total exposure time doesn’t seem to match the data in the RDSR file. Hoewever a lot of data is simply correct on import in OpenREM,  anyone who knows what I have to change ?

 

I marked the imported data from Openrem with yellow in the dsrdump of the files (first 3, there are more)

That would indeed be useful, as we will need to adapt OpenREM to address this issue.

The problem for the DAP and Dose (RP) is that the units that should be used are defined in the standard, so even though the different units are declared in your RDSR according to the dump you have shared, OpenREM doesn't check the units declaration because there should be no point!

For example:
Row 18 of TID 10003, Dose Area Product: UNITS = EV (Gy.m2, UCUM, "Gy.m2"). This compares with your DICOM dump showing units of dGy.cm2 - a factor of 100,000 different.

Row 1 of TID 10003B, Dose (RP): UNITS = EV (Gy, UCUM, "Gy"). This compares to mGy in your DICOM dump, explaining the factor of 1000. It also uses a non-standard definition for reference point, though that is allowed :-)

Exposure time is right - 67.2 ms, 261 ms and 391 ms.

The one that confuses me is total fluoro time. It is correctly stated in seconds, so I don't know where the factor of ten comes in.

Have you checked the export to see if the total fluoro time is wrong there too?

Is this perhaps what Canon are using?

Yes it was the correct unit to use for the Radiation Dose Module of the MPPS, as linked, though thankfully that module has been retired as of 2017d as the RDSR is the proper place for that information, not MPPS. Existing implementations are still valid though.

It is also the correct unit for the DX IOD: http://dicom.nema.org/MEDICAL/dicom/current/output/chtml/part03/sect_C.8.7.8.html - which is why we expect dGy.cm2 in our DX headers. And also why Carestream insist on displaying in dGy.cm2, which is just annoying!

Neither of those make it right to use it in the RDSR though!

Thanks for you answer, information in both OpenRem and the dicom RDSR of the exam gives a cumulative 64 sec indeed instead of the total duration of 640.00 mentioned in openrem

 

Exam time has nothing to do with it ? This is from 11.52.25 – 12.03.09 => 7.35 + 3.09 min = 10.44 min = 644 sec will be coincidence.

 

Possibly remarkable is the point and comma issue in Openrem here:

 

Wereas for example the first run:

Could this be not a factor 10 in time difference but a factor 1000 (or other). This comma is not in de dicom RDSR, maybe units here too ?

 

I’ll yet have to check the export itself on the device to see if it is wrong and let you know.

 

I can also contact Canon about this issue and find out what they can do ? We have a XA (Infinix) and Toshsiba CT (Prime) here too J

Yes, I didn't spot the mixed use of comma and dot. But as a thousand indicator, it doesn't make sense either, and doesn't tally with the pulse rate and mA. I think it is more likely that your regionalisation settings are giving you the comma, appropriately, but possibly when we present the dose information we are transforming it for human consumption (Gym2 doesn't look nice), and that seems to prevent the regionalisation kicking in. I haven't looked at the template to check!

it really would be helpful to get hold of and RDSR from this unit - do you have any QA images you could send me?
